Abstract

WP-06 in the VAT-Trap / SCVC White Paper series. The Ten-Ten Protocol proposes ten minutes of progressive resistance training and ten minutes of interval work each day, on a base of habitual walking, as a dose small enough to be sustained for decades. This paper examines the interval component: what the evidence supports for visceral fat, cardiorespiratory fitness and glycaemic control, and what it does not yet support for reducing or withdrawing from GLP-1 receptor agonist therapy. The paper sets out three distinct applications — standalone visceral fat reduction, incretin dose minimisation, and withdrawal support — and argues that the strongest case is the first, since cardiorespiratory fitness predicts mortality more powerfully than smoking or diabetes and is not improved by incretin therapy. It presents the mechanistic case that a short bout initiates signalling lasting far longer than the session itself, and shows that the randomised evidence closest to the withdrawal question used substantially more supervised exercise, leaving the Ten-Ten prescription untested. Nine claims are separated in an explicit hypothesis register graded from strong to untested, and the paper states throughout where its evidence runs out.
