Abstract

Abstract: Conventional pharmacotherapy for gastrointestinal (GI) and hepatobiliary malignancies—specifically hepatocellular carcinoma (HCC) and metastatic colorectal cancer (CRC)—remains throttled by severe systemic toxicities, off-target destruction of healthy hepatic tissue, and rapid emergence of acquired drug resistance driven by clonal heterogeneity and secondary mutations. In contrast, natural mammalian evolution has engineered multi-layered biomolecular complexes that selectively eradicate malignant cells while leaving healthy differentiated tissues completely intact. Synthesizing our foundational treatise on human milk-derived α-lactalbumin-oleic acid complexes (HAMLET / synthetic Alpha1H, Zenodo 2026) with recent empirical validation from North African patient cohorts (Haider & Ahmed, Zenodo 2025), this monograph establishes an advanced clinical and biophysical translational roadmap for deploying Alpha1H against gastrointestinal malignancies. We formalize four core therapeutic paradigms: (1) Universal Tumor Selectivity: Cross-population evidence confirms that HAMLET-like complexes selectively destroy HCC and CRC cell lines in a dose-dependent manner (SI >> 50) across diverse human genetic backgrounds without hepatocyte or intestinal epithelial toxicity; (2) The Multi-Organelle Hydra Mechanism: Alpha1H exploits tumor-specific outer-leaflet phosphatidylserine (PS) exposure to trigger receptor-independent membrane translocation, executing coordinated destruction across four cellular compartments: ribosomal translation arrest, irreversible mitochondrial permeability transition pore (mPTP) opening, lethal endoplasmic reticulum stress, and non-canonical nuclear translocation with direct core-histone (H2B/H3/H4) binding, completely circumventing p53 mutations and Bcl-2/caspase blocks; (3) HMO Multi-Layered Synergy: Co-administration of Human Milk Oligosaccharides (HMOs) eliminates Single Points of Failure (SPOF) through glycobiological decoy mechanics that weaken the tumor glycocalyx and metabolic axes that generate short-chain fatty acids (butyrate), inducing epigenetic histone deacetylase (HDAC) inhibition; and (4) Nanocarrier DDS & Immuno-Oncology Integration: We formulate pH-responsive lipid nanoparticles (LNPs) and galactose-functionalized polymeric micelles for targeted oral and intravenous delivery, coupling Alpha1H-induced Immunogenic Cell Death (ICD) with immune checkpoint inhibitors (anti-PD-1/PD-L1) to convert cold gastrointestinal tumors into durable immunogenic eradication.
