Abstract

Background: Vātarakta is a complex, progressive clinico-pathological entity in Ayurveda characterized by the simultaneous aggravation and profound interaction of Vāta Doṣa and Rakta Dhatu. While contemporary biomedical science often correlates it broadly with gout and inflammatory arthropathies, the classical Ayurvedic treatises (Bṛhat-trayī) elucidate a highly specific pathogenesis rooted in Mārga Avarodha (channel obstruction) and sequential Doṣa-Dūṣya Saṃmūrchana. Despite extensive descriptions in classical texts, a critical, comparative synthesis prioritizing Charaka Saṃhitā’s foundational pathophysiological architecture remains inadequately explored in modern literature .Objective: To critically analyse and conceptually synthesize the Nidāna (etiology), Samprāpti (pathogenesis), Bheda (classification), and Chikitsā (management) of Vātarakta as described in the Bṛhat-trayī, with a special emphasis on Charaka Saṃhitā, and to evaluate its contemporary relevance to inflammatory arthropathies. Materials and Methods: A qualitative textual analysis and comparative conceptual synthesis were conducted. Primary data were extracted from the Charaka Saṃhitā, Suśruta Saṃhitā, and Aṣṭāṅga Hṛdaya alongside their authoritative commentaries. Secondary data included Scopus and PubMed-indexed peer-reviewed publications. The evidence was hierarchically categorized to strictly delineate classical textual statements from modern pathophysiological interpretations. Results: Charaka Saṃhitā establishes a unique paradigm wherein Vātarakta is neither exclusively a Vāta disorder nor solely a Rakta disorder, but a discrete entity arising from bidirectional pathological obstruction (Avarodha). The comparative analysis reveals that while Charaka emphasizes the systemic metabolic and circulatory interaction (Vāta-Rakta Saṃmūrchana), Suśruta elaborates on structural and surgical dimensions (Raktamokṣaṇa), and Vāgbhaṭa provides a highly structured clinico-therapeutic synthesis. A reconstructed Samprāpti model links dietary indiscretions directly to Srotoduṣṭi and Sthāna Saṃśraya in the distal joints. Conclusion: Vātarakta represents an autonomous, multidimensional disease model. Charaka’s conceptualization emphasizes the restoration of Vāta gati (movement) and Rakta prasādana (blood purification) through stage-specific Śodhana and Śamana. While presenting overlapping phenotypes with modern gout (hyperuricemia), Vātarakta encapsulates a broader spectrum of neuro-vascular and inflammatory pathologies.
