From Clues to Clarity: A Simplified Approach to Connective Tissue Disorders for Dermatologists

Abstract

Connective tissue diseases (CTDs) represent a clinically diverse group of autoimmune disorders marked by immune-mediated damage to the skin and internal organs. Among the most frequently encountered CTDs in dermatology practice are systemic lupus erythematosus, systemic sclerosis, dermatomyositis, mixed CTDs and antiphospholipid antibody syndrome. Cutaneous features often precede systemic findings and may serve as the first diagnostic clue. Most of the time, these features may not present simultaneously but may appear gradually or sequentially, often evolving over time in the same patient. The skin manifestations of CTDs include malar and discoid rashes, photosensitivity, oral ulcers, Gottron’s papules, heliotrope rash, Raynaud’s phenomenon, livedo racemosa and sclerotic plaques, among others. This article outlines a practical, stepwise approach to CTDs. It should begin with a high index of clinical suspicion based on cutaneous and systemic signs. Dermatologic findings, when interpreted with associated symptoms such as fatigue, low-grade fevers, arthralgias, dysphagia or myopathy, can narrow the differential diagnosis early. Subsequent laboratory evaluation, including baseline inflammatory markers and antinuclear antibody testing, followed by specific autoantibody profiling, further guides towards the diagnosis and management. Management involves integration of general supportive care and immunosuppressive therapy. Dermatologists play a central role not just in early diagnosis but also in monitoring disease course and treatment response. This review aims to simplify CTD recognition and workup from a dermatologic standpoint, especially for clinicians, by translating textbook knowledge into a structured, bedside-friendly framework.
