ROLE OF SHODHANA AND STHANIKA CHIKITSA IN KAPHAJA YONIVYAPADA: A CASE REPORT WITH CLINICAL CORRELATION TO VULVOVAGINAL CANDIDIASIS

Abstract

Kaphaja Yonivyapada, described in the Charaka Samhita, is characterised by symptoms such as Kandu (itching), Picchila, Pandu/Shweta Srava (mucoid or white vaginal discharge) resulting from the vitiation of Kapha dosha1. Clinically, it can be correlated with Vulvovaginal Candidiasis. This case report presents the Ayurvedic management of a reproductive-aged female who presented with recurrent vulvovaginal and perianal itching and vaginal dryness associated with curdy white vaginal discharge. Her sleep was also disturbed due to stress and vaginal symptoms. Based on the clinical features, the condition was diagnosed as Kaphaja Yonivyapada. The patient underwent a comprehensive Ayurvedic treatment protocol including Sarvanga Utsadana, Bashpa Sweda, Sthanika chikitsa - Yoni Prakshalana with Triphala and Nimba Kashaya, Snehapana with Kalyanaka Ghrita, followed by Virechana Karma using Trivrut Lehya, followed by Samsarjana Krama and oral Shamana therapy. Marked improvement was observed in vulvovaginal itching and curdy white discharge, with sustained symptomatic relief during follow-up and no reported adverse events. This report highlights the potential role of classical Ayurvedic interventions, particularly Shodhana therapy and Sthanika Chikitsa in the effective management of Kaphaja Yonivyapada.
